Gastric
bypass is one of the most common weight loss operations performed
today and has helped many obese individuals. However, even
the most successful gastric bypass patients occasionally regain
weight. One of the potential causes can be pouch or stoma (the
connection between your stomach and small intestine) dilation
that can occur years after the original surgery. This may allow
patients to tolerate larger portions of food during meals with
eventual weight regain.
In the past, there were few treatment options to decrease
the size of the pouch and stoma. Revision surgery is sometimes
used, but this can be more difficult than the original gastric
bypass surgery with increased complication rates due to scarring
and adhesions. Many patients may decide against surgery and
live with their increased weight. However, we now offer an
alternative to traditional revision surgery. The ROSE Procedure
(Reconstructive Obesity Surgery, Endolumenal) is a new incisionless
procedure that can restore the size of the pouch and stoma
to the original post-surgery dimensions.
The ROSE procedure is performed entirely through the mouth
using a small flexible endoscope and surgical tools without
making any external incisions on the body. The surgeon advances
the scope through the mouth into the patient’s stomach
pouch. The flexible scope has multiple channels that allow
the surgeon to advance multiple surgical tools into the pouch.
The surgeon then makes multiple folds in the stoma and pouch
to reduce its overall diameter and volume. This can effectively
restore the patient’s pouch and stoma to the original
post surgical size.
Patients will require an initial full evaluation by the
surgeon, nutritional and dietary evaluations, and an upper
endoscopy to assess the stomach pouch and stoma. The ROSE
procedure is typically performed as an outpatient procedure
in a hospital operating room. Most patients will be discharged
home the same day or in less than 23 hours. Because there
are no external incisions, most patients have a very quick
recovery.
The ROSE procedure offers other important advantages to patients,
including a reduced risk of infections and associated complications,
less post-operative pain, and no abdominal scars. Most patients
will have little discomfort after the procedure, but some
patients may report a sore throat for a short time. Since
this is a new procedure, long-term weight loss statistics
are not yet published, but preliminary data show that most
patients regain the feeling of “fullness” and
experience weight loss after the procedure. At Texas Center
